The OMSC Awards and Recognition program is a chance to honour our teammates and supporters for the swimming achievements, volunteer contributions and inspiration they have offered us over the 2011-2012 swimming season. This year we are asking all OMSC members to participate by nominating who they think most deserves one of our seven awards.

The OMSC Nomination form 2011-2012 above may be submitted in one of three ways:
1. Online simply by completing the highlighted areas and pressing the “submit” button at the top of the form. Choose “send” and follow the on screen instructions;
2. Save the form to your computer, complete and attach to an email addressed to awards@okmasters.com;
3. Or print and complete by hand. Printed copies can be placed in the “nomination envelope” found in the attendance binder at all swim practices.
Please take a moment to honour those who contribute so much to our club. Nominations will be accepted until October 10th, 2012.
